My name is Katy Hernandez MSN, FNP-BC a primary care provider at Salem Wellness Clinic serving the entire family from children to older adults. My emphasis is on patient-centered healthcare where I empower and support patients to be active members of their healthcare decisions to ensure the best outcomes to last a lifetime. I am passionate about patient education and ensuring that my patients are not only an active part of their healthcare decisions but are educated in order to make the best decisions to lead a healthy life. I am an Oregon native but have lived in Washington, Alaska, and New York over the past 10 years. I received my Family Nurse Practitioner degree from Frontier Nursing University in 2021. I enjoy working in primary care for all ages. I am a devoted wife and mother to a two-year-old daughter. I love outdoor activities in any season, exploring new recipes, and spending time with my family.
